What you will be doing
Triage and Assessment
- Perform comprehensive triage of referrals and medical records to determine urgency, risk, and appropriate care pathway
- Deliver telephone and video triage assessments using internal policies and clinical best practice
- Identify exclusions, contraindications, and safeguarding concerns, escalating in line with Montu policy and national guidance
Care Coordination
- Maintain accurate, timely clinical records
- Signpost, refer, and escalate internally and externally as required
Service Development and Quality
- Contribute to or lead pathway audits, SOP development, and outcome measurement
- Support the creation of patient education materials and digital triage tools
- Champion clinical governance, incident reporting, and continuous improvement
- Play an active role in developing the physiotherapy function across the business
What you need
Essential
- HCPC-registered Physiotherapist; CSP member or equivalent
- Minimum 5 years post-registration experience, with at least 2 at Band 6 or above
- Demonstrable expertise in remote and telephone triage and safety-netting
- Strong clinical reasoning and confident autonomous decision-making
- Clear written and verbal communication, including the ability to translate complex clinical information for patients
- Comfortable in a fast-paced, tech-enabled environment including EMRs, telehealth platforms, and digital PROMs
- Sound understanding of safeguarding, GDPR, and clinical governance
Desirable
- Experience in pain management, cannabis-based medicinal products, or complementary therapies
- Background in a start-up, scale-up, or high-volume healthcare setting
- Experience developing SOPs, leading audits, or contributing to service redesign.
